Components of the TLC program

The TLC Diabetes program is made up of four parts:

1. Weekly telephone calls to the automated TLC Diabetes system
2. Follow-up calls from the TLC Coordinator to see how you're going
3. A handbook, which provides information about how to use the system and tips for diabetes management
4. The AliveConnect kit to send your blood glucose readings to the TLC system
